The inspection resulted in a critical deficiency on inadequate cleaning validation as well as eight (8) major findings.
Key facts · Basis: official source record kept by the collector
Major findings: eight, covering container closure integrity testing and delayed correction of long-standing issues, aseptic process simulation, process validation, and qualification of manufacturers and suppliers of critical starting materials
Non-compliant scope: aseptically prepared small volume liquids, terminally sterilised large volume liquids, secondary packaging
Action: no batches manufactured at the site should be released to EEA markets while the non-compliance stands or until it is superseded by an EU GMP certificate
Insight · Editorial insight
Rating cleaning validation as critical and then bundling container closure integrity, aseptic process simulation and process validation into a non-compliance statement shows sterile sites being judged on the whole of their sterility assurance system rather than on isolated findings. Quality and regulatory teams running or outsourcing sterile manufacture should check whether cleaning validation and container closure integrity testing are sitting as long-standing open findings, and whether the correction history is documented.
Check points
Rationale for cleaning validation sampling locations and adequacy of recovery data
Container closure integrity test method and frequency, and CAPA progress on long-standing findings

1NON-COMPLIANT MANUFACTURING OPERATIONS
1.1Sterile products
1.1.1Aseptically prepared (processing operations for the following dosage forms)

1.1.2Terminally Sterilised (processing operations for the following dosage forms)
1.1.2.1Large volume liquids
1.5Packaging
1.5.2Secondary packaging
Nature of non-compliance
Original · EudraGMDP
The inspection resulted in a critical deficiency on inadequate cleaning validation as well as eight (8) major findings. These dealt with inadequate control of identification labels inside the manufacturing area, inadequate management of visual inspection activities, sterilized product passing through an area where un-sterilized bags were waiting to be autoclaved, inadequate container closure integrity testing and delayed action to correct this very long standing issue, process validation inadequacies, deficient complaint management, deficient management of aseptic process simulations as well as insufficient qualification of manufacturers and suppliers of critical starting materials and no reassurance that primary packaging materials for ophthalmic products were free from contamination. There were also 24 other deficiencies, in addition to the critical and major findings. All these observed deficiencies raise concerns that the approach to the manufacturing of sterile products is not adequately managing inherent risks pertaining to sterile processes and is not in line with applicable EU GMP requirements. As a result of this outcome, the Inspection Review Group (IRG) at the Malta Medicines Authority has met and decided that a Statement of Non-Compliance with the principles and guidelines of EU Good Manufacturing Practice is to be issued for the site.
Action taken/proposed
Original · EudraGMDP
Prohibition of supply No products batches manufactured at the site should be released to EEA markets until this noncompliance remains in place or is superseded by an EU GMP certificate. Others Member states should consider not initiating or authorising new marketing authorisations with this site listed as a finished product manufacturer until the recommended non-compliance statement to be issued remains in place or is not superseded with a GMP certificate. The following applications are known to be ongoing: Sodium Chloride Intravenous Infusion – 0.9% w/v, nPVC bag 0.9% w/v Czechia (RMS), Germany (CMS), Spain (CMS) & Portugal (CMS) Ref. CZ/H/1558/001/DC Timolol 5 mg/mL Eye Drops Solution 5 mg/mL, 5 ml LDPE bottle Spain , Malta, Portugal, Hungary DCP slot requested in September 2025.
Further comments
Original · EudraGMDP
Since the site does not have any current valid EU-GMP certificates, there should not be any medicinal products manufactured at the site present in EEA markets. A teleconference has therefore not been proposed in view of this.
[Warning Letter · FDA]R3 Medical Companies — Unvalidated aseptic process, six CGMP citations
Evidence BFDASignal Low · T1🧬 BiologicsCGMP
FDA's Center for Biologics Evaluation and Research issued a warning letter to R3 Medical Companies, covering manufacturer Bello and distributor Regen. Alongside treating the umbilical cord and cord blood derived products as unapproved new drugs and unlicensed biologics, the letter cited six violations of 21 CFR Parts 210 and 211, including no aseptic process validation, freezing of samples before sterility testing, and expiry dating assigned without stability data.

Center for Biologics Evaluation and Research (CBER) · 08/14/2026
Key facts · Basis: official index plus supporting sources
Violations: six CGMP citations - no validation of the aseptic process and no validated cleaning and disinfection of the biological safety cabinet (21 CFR 211.113(b), 211.42(c)(10)(v))
Sterility testing: samples were frozen and thawed before testing, which can destroy or injure microorganisms so that contamination present before freezing may not be reliably detected (21 CFR 211.160(b))
Expiry dating: a 24-month expiry was assigned without stability testing data, and quality unit responsibilities and procedures were not documented (21 CFR 211.166(a), 211.22(d))
Action: written response within 15 working days, with seizure or injunction possible if the violations are not corrected
Insight · Editorial insight
A letter headed by unapproved drug status that also carries CGMP violations across the whole sterility assurance system shows that a site's GMP standard is judged independently of the dispute over how the product is classified. The point that freezing samples before sterility testing reduces detection is not confined to cell therapy but reaches any sterility test run on frozen samples, and a footnote treating the distributor as a manufacturer because it labels, stores and releases product transfers directly to contract manufacturing and own-label distribution arrangements.
Check points
Whether sterility test samples are frozen and thawed, and the method suitability evidence for the effect on microbial recovery
Stability data supporting every assigned expiry date, and whether any expiry was set without it
Whether an own-label distributor that labels, stores or approves release is managed as being subject to GMP

Key facts · Basis: official source record kept by the collector
Violation: breach of the compliance rules on recall and disposal that apply to a recall obligor
Sequence: recall completion reports filed on 14 and 29 April 2026, then the affected lot shipped again in early May 2026, before closure was notified
Notification: the Daejeon regional office confirmed closure of the recall on 19 and 22 June 2026
Action: 15-day suspension of manufacturing for the affected items (10 to 24 September 2026), covering four dried ginkgo leaf extract tablet products
Insight · Editorial insight
Suspending manufacture because product was shipped again on the view that filing the completion report ended the recall confirms that it is the regulator, not the recall obligor, that decides when a recall closes. Teams running recalls should confirm that the affected lots stay locked in a shipment hold until the written notice from the competent regional office is in hand.
Check points
Procedure blocking shipment between filing a recall completion report and receiving written notice from the regional office
Authority and records for releasing recalled lots from quarantine and returning them to distribution
Retention and traceability of regulator notices closing a recall

Insight · Editorial insight
Grading the suitability of the microbial limit test method as major in a pre-approval inspection of an overseas site shows that the validity of the method used to judge a specification is examined alongside the specification itself. Quality teams sourcing finished product from overseas sites should confirm that method suitability data, microbial limit testing included, is in place before the inspection rather than after it.

Key facts · Basis: official source record kept by the collector
Violation: the firm's own standard 'GMP documentation' (document no. SOP-G-0201) requires every record to be written and completed at the same time as the work, but the manufacturing steps were not recorded contemporaneously
On-site check: completed weighing records for raw materials could not be shown while the mixing step was running, and pre-work checks, equipment checks and running records, and verification of the raw materials charged into the process had not been done
Action: a penalty surcharge of KRW 19,350,000 in place of a 15-day suspension of manufacturing for the dosage form
Calculation: the daily suspension surcharge for the 12 products with production in the reference year, out of the 14 cataplasm products, multiplied by 15 days
Insight · Editorial insight
Treating a contract manufacturer's failure to follow its own standard as a breach of contract management responsibility, and carrying it through to a surcharge in place of a dosage form suspension, signals that a loss of contemporaneity in records is read as a failure of manufacturing control rather than a documentation defect. Quality teams manufacturing through contract arrangements should look at whether they verify contemporaneous completion of the contractor's manufacturing instruction and record on the floor, not by collecting paperwork afterwards.
Check points
On-site verification that manufacturing instructions and records are completed at the same time as the work
Contract giver's routine audit of the contractor's compliance with its own standards
Completeness of pre-work checks, equipment running records and verification of raw materials charged into the process

Advice: stop using the affected products, consult a healthcare provider for any health concerns, and do not buy or use unauthorised health products
Insight · Editorial insight
A product presented as herbal in which a prescription drug ingredient was detected, escalating to the highest risk class, shows that the recall class follows what is actually found rather than what the label declares. Quality teams handling botanical and herbal materials should confirm that the possibility of undeclared synthetic adulterants is captured as a test in the supply chain.
Check points
Screening for undeclared synthetic adulterants across the botanical and herbal supply chain
Whether identification testing includes methods able to detect unapproved analogues

Reason: acid-insoluble ash
Type: mandatory recall
Recall order date: 25 August 2026
Product: a herbal (crude drug) product
Insight · Editorial insight
A mandatory recall on the acid-insoluble ash test alone shows that a purity indicator is not a secondary specification but can trigger a recall on its own. Acid-insoluble ash is generally associated with soil and foreign matter carried in with the material, so quality teams handling crude drugs should look at whether the test is performed at goods receipt and at how the supplier is managed.
Check points
Whether acid-insoluble ash is tested at goods receipt of crude drug materials, and the acceptance criteria
Supplier assessment of foreign matter control at growing and collection

Insight · Editorial insight
A mandatory recall of a herbal preparation on the ground of sulfur dioxide shows that residues entering at the material stage lead straight to a recall, not only the finished product specification. Sulfur dioxide is generally associated with treatments used in drying and storage of crude drugs, so quality teams should confirm the supplier's processing history and the sulfur dioxide test applied at goods receipt.
Check points
Sulfur dioxide test and specification applied at goods receipt of crude drug materials
Procedure for confirming each supplier's drying and storage treatment history

Check points
Whether the acceptance criteria for description of crude drugs match the specification document
Training of assessors and retention of description test records

Check points
Element-by-element heavy metal test panel and specification for crude drug materials
Traceability of material origin by growing region and the supplier requalification cycle
