The classifications (NAI, VAI, OAI) FDA assigned after inspecting drug manufacturing sites. This counts how each inspection ended, not individual findings.

The yearly table above uses the fiscal year (starting in October); this one uses the date the inspection actually ended. Each bar is the share of that quarter's inspections classified OAI — even bar lengths mean the severity of classifications was stable over the period.
